Speaker 2:All right, so today we're going to be talking all about Motiva breast implants. Sounds good. We have a bunch of stuff from Dr Roy Kim.
Speaker 1:Yeah.
Speaker 2:And then also some stuff from Motiva themselves.
Speaker 1:That's perfect, because then we can kind of get the plastic surgeon's perspective with Dr Kim and then also the company's more technical perspective.
Speaker 2:Yeah, for sure.
Speaker 1:With the Motiva official information.
Speaker 2:Yeah, so Motiva calls itself like the sixth generation of breast implants. Oh, wow, so it seems like they're trying to be like the most advanced breast implant technology out there, right, and so they say that they've been used in over three million procedures worldwide. But they just got FDA approval in the US in September of 2024.
Speaker 1:So very new for the US, but really well established globally.
Speaker 2:Yeah, yeah.
Speaker 1:And I think one of the things that people are really interested in is they really try to focus on safety and having a more natural result.
Speaker 2:Yeah, I saw Dr Kim talking about that a lot in his videos.
Speaker 1:Yeah.
Speaker 2:Okay so let's get into some of the science behind Metiva. Okay so Dr Kim talks about silk surface technology.
Speaker 1:Yeah, that's one of their big ones. What?
Speaker 2:is that it sounds very high tech.
Speaker 1:It is. It's all about minimizing the body's reaction to the implant.
Speaker 2:Okay.
Speaker 1:Because traditionally, with some breast implants, what can happen is it can trigger a reaction that leads to scar tissue buildup around the implant yeah, and that's called capsular contracture. Okay, and what happens is the implant starts to feel hard and look distorted, and you don't want that.
Speaker 2:Yeah.
Speaker 1:That's not ideal.
Speaker 2:That sounds bad.
Speaker 1:So what they try to do is they make this surface called silk surface, and it's incredibly smooth.
Speaker 2:Oh, wow.
Speaker 1:Think of it like a tiny microscopic silk scarf, so it's actually nanotextured. But the average roughness is just four nanometers, wow, so it's super smooth and that helps the implant integrate better with the body's tissues, so you reduce the chances of that scar tissue forming.
Speaker 2:Gotcha, so it's just less irritation to the body.
Speaker 1:Less friction, less irritation, a much happier implant.
Speaker 2:And so Dr Kim was talking about how they have really low capsular contracture rates.
Speaker 1:Yes.
Speaker 2:Like less than 1%.
Speaker 1:In some cases, yes, and that's significantly lower than what we've seen with some of the more traditional implants.
Speaker 2:Wow.
Speaker 1:And that's huge because that could mean potentially fewer revision surgeries, right, and, of course, more satisfied patients. Yeah, that makes sense In the long run.
Speaker 2:Okay, so of course, more satisfied patients. Yeah, that makes sense In the long run. Okay, so that's the silk surface, so that's like the outer part of the implant.
Speaker 1:Right.
Speaker 2:What about, like the material on the inside of the implant?
Speaker 1:Right. So Mativa also has something called Progressive Agile Plus and Progressive Agile Ultima.
Speaker 2:Wow, these are very high tech names. They are Okay.
Speaker 1:But basically what this gel is designed to do is mimic the feel of natural breast tissue.
Speaker 2:Oh, wow.
Speaker 1:So it moves and responds a lot more like the real thing.
Speaker 2:So it feels more natural.
Speaker 1:Exactly it's not just about looking natural. It's also about the feel.
Speaker 2:Yeah, that's what I saw in a lot of Dr Kim's videos, like people were commenting about how they feel so natural.
Speaker 1:Right.
Speaker 2:And I think that this is probably why.
Speaker 1:Exactly, and it also helps with the problem that we see sometimes with more traditional implants, which is something called rippling.
Speaker 2:Oh, what is that?
Speaker 1:So rippling is when the implant kind of wrinkles or folds.
Speaker 2:Okay.
Speaker 1:And it's visible through the skin.
Speaker 2:Oh no.
Speaker 1:Yeah, so it's not a good look.
Speaker 2:Not a good look.
Speaker 1:But Motiva's gel is designed to be super cohesive. Okay, so shape better and you minimize that rippling effect.
Speaker 2:Gotcha Okay. So silk surface on the outside.
Speaker 1:Yes.
Speaker 2:Progressive Eagle on the inside.
Speaker 1:Right.
Speaker 2:Okay, they really have thought of everything.
Speaker 1:They have and they're addressing so many of the concerns that I think have plagued breast implants historically. Yeah, and they have all these other cool things too.
Speaker 2:Like what.
Speaker 1:Do you remember that blue seal barrier we were talking?
Speaker 2:about oh, yeah, yeah.
Speaker 1:That's a big safety feature that Dr Kim highlights.
Speaker 2:Okay, so what is that exactly?
Speaker 1:It's basically a thin blue layer that's part of the implant shell.
Speaker 2:Okay.
Speaker 1:Think of it as like a built-in safety check.
Speaker 2:Okay.
Speaker 1:So during surgery, the surgeon can visually confirm that the blue layer is intact oh wow which means the implant hasn't been damaged or compromised. So it's just an added layer of reassurance. Yeah, peace of mind For both the surgeon and the patient.
Speaker 2:Yeah, for sure. Especially when you're going into surgery, you want to make sure everything is good to go.
Speaker 1:Absolutely.
Speaker 2:Okay, so they have that. And then there's also I think it's called true monoblock shell. Yeah, Is that what it's called?
Speaker 1:Yes, true, monoblock shell.
Speaker 2:What is?
Speaker 1:that. So basically, it means that the shell, the patch, which is the part that seals the filling hall and the gel they're all unified into a single unit during manufacturing.
Speaker 2:Okay.
Speaker 1:So this eliminates any potential weak points where leaks or tears could occur. Oh, that makes sense, it's like creating this seamless, super strong structure.
Speaker 2:Yeah, okay, so they've really thought of everything.
Speaker 1:They're combining innovative materials, clever design, rigorous manufacturing.
Speaker 2:Yeah.
Speaker 1:To try to make an implant that's potentially safer, more durable and looks and feels more natural.
Speaker 2:Okay, so they had different shapes and sizes, obviously right.
Speaker 1:Yes.
Speaker 2:What are the main types?
Speaker 1:So, when it comes to the shape, you have two main choices round and ergonomics.
Speaker 2:Okay, so round are probably what people think of traditionally when they think of breast implants Exactly the classic round implants and they give you that fullness in the upper part of the breast. Okay.
Speaker 1:And they create more cleavage Gotcha of the breast. Okay, and they create more cleavage gotcha. So if you're looking for a noticeable enhancement, rounds might be the way to go okay more of that bombshell look okay, and what about the ergonomics?
Speaker 1:so the ergonomics. This is where things get really interesting okay because these implants are designed to adapt to your body's natural movements oh, wow so when you're standing or sitting they take on a teardroprop shape for a subtle lift and natural profile, but when you lie down they flatten out just like real breast tissue would.
Speaker 2:Oh wow.
Speaker 1:So it's all about achieving that natural look and feel.
Speaker 2:So it changes shape.
Speaker 1:It does, based on your position.
Speaker 2:So they move with your body.
Speaker 1:Exactly.
Speaker 2:That's so cool.
Speaker 1:Yeah, and patients love this feature.
Speaker 2:Yeah, I can imagine.
Speaker 1:It's all about that natural look and feel.
Speaker 2:Okay, so we've talked about all the science and how they're made and everything.
Speaker 1:Right.
Speaker 2:Let's talk about some of like the more practical side of things, okay, like how long do these implants last?
Speaker 1:Okay.
Speaker 2:And what kind of warranty do they offer?
Speaker 1:Those are great questions.
Speaker 2:Yeah.
Speaker 1:So no implant lasts forever.
Speaker 2:Right.
Speaker 1:But Motiva claims that their implants are designed for longevity.
Speaker 2:Okay.
Speaker 1:And they could last potentially 10 to 15 years or more without complications. And to back that up they have a warranty program called the mode of a health program, and it provides lifetime coverage for rupture. So that means if the implant shell breaks, they'll replace it for free.
Speaker 2:So that's for life.
Speaker 1:That is for life. Now, what about other potential issues?
Speaker 2:Yeah, like what if you get capsular contracture?
Speaker 1:Yes, so they have coverage for that as well. Okay, so the basic warranty includes 10 years of coverage for capsular and contracture Right, but only for the severe cases.
Speaker 2:Okay, which?
Speaker 1:are called Baker grades the third and fourth.
Speaker 2:Oh yeah, Dr Kim talked about that.
Speaker 1:Yes, so Dr Kim explains those grades in one of his videos, but basically it's how firm and distorted the breast becomes due to the scar tissue. Okay so the warranty kicks in if it gets really bad.
Speaker 2:Gotcha Okay, and do they give you any financial assistance if you need surgery?
Speaker 1:They do so. They offer $2,000 towards surgical fees for two years if you need a reoperation due to capsular contracture. That's good, and that's just with the basic warranty.
Speaker 2:Okay, and is there like an enhanced warranty or something?
Speaker 1:There is For an extra $250,. You can upgrade to their Motiva Health Program Plus.
Speaker 2:Okay, and what's so great about that?
Speaker 1:So, first off, it extends the coverage for capsular contracture to five years. Okay, and then, if you need surgery due to capsular contracture, they increase the financial assistance to $2,500. Oh, wow, so that's quite a bit more.
Speaker 2:That's significant.
Speaker 1:Yes.
Speaker 2:Okay, so they have that. And then there's also the Femtech Promise. Oh yes, what is that?
Speaker 1:This is the one that I love.
Speaker 2:Okay.
Speaker 1:So it's basically financial assistance for implant removal within the first two years.
Speaker 2:Even if you just change your mind.
Speaker 1:Really.
Speaker 2:Yes, no questions asked.
Speaker 1:That's a pretty big deal.
Speaker 2:It is.
Speaker 1:It seems like they really stand behind their product.
Speaker 2:Yes, they're very confident in it.
Speaker 1:Okay.
Speaker 2:And they want their patients to be happy.
Speaker 1:Yeah, because, like I said before, going into surgery is a big deal.
Speaker 2:It is.
Speaker 1:You want to make sure you're making the right decision.
Speaker 2:Absolutely.
Speaker 1:Okay, so we've talked about Motiva a lot.
Speaker 2:Right.
Speaker 1:But what about? Like the other brands that are out there?
Speaker 2:Like.
Speaker 1:Mentor Allergan Sientra.
Speaker 2:Right, so those are all the big names that you hear about.
Speaker 1:Yeah.
Speaker 2:And they all offer similar shape options.
Speaker 1:Okay.
Speaker 2:So Round and Teardrop, and they all have FDA approval, but when it comes to the specifics of their warranties, yes, that's where things get a little bit different.
Speaker 1:Okay, so what are the main things that you should look for when you're comparing warranties?
Speaker 2:You want to pay attention to what's covered, the length of coverage and the amount of financial assistance.
Speaker 1:Right.
Speaker 2:And then you want to consider your individual needs and risk tolerance. Yeah, like, if you're really concerned about capsular contracture, you want to make sure that they cover that.
Speaker 1:Exactly.
Speaker 2:Okay, so let's talk about rupture coverage first.
Speaker 1:Okay, so the good news is all four brands Motiva, allergan, mentor and Sientra. They offer lifetime coverage for implant replacement if a rupture occurs.
Speaker 2:Okay, that's good.
Speaker 1:But there is some variation in the financial assistance they provide for the surgery itself.
Speaker 2:Oh, so they might cover the implant, but not necessarily the cost of actually getting it replaced.
Speaker 1:Right, you got to read the fine print, yeah gotcha. All right, so let's move on to capsular contracture coverage.
Speaker 2:Yeah.
Speaker 1:So Motivo's basic warranty covers it for 10 years. Allergan's confidence plus warranty also offers 10 years. Uh-huh Mentor's basic warranty also offers 10 years Mentor's basic warranty also offers 10 years. Okay. However, Mentor's enhanced warranty and Ciantra's Platinum 20 warranty extend that coverage to 20 years.
Speaker 2:Wow.
Speaker 1:So there's a little bit more variation there.
Speaker 2:Okay.
Speaker 1:And just like with rupture coverage, the amount of financial assistance for a reoperation due to capsular contracture also different right.
Speaker 2:So remember, Motiva's basic warranty is $2,000 for two years. Their enhanced warranty is $2,500 for five years. Allergan's Confidence Plus provides $2,000 for two years.
Speaker 1:Okay.
Speaker 2:Now Mentor's warranties are a little more generous. Okay, they offer $3,500 for 10 years. Wow, they offer $3,500 for 10 years. And then Sientra's Platinum 20 stands out with $2,000 for two years for a primary augmentation, and then, if it's a revision augmentation, they give you an additional $2,500 for two years.
Speaker 1:Okay, so it seems like there are a lot of differences between these warranties.
Speaker 2:There are a lot of nuances.
Speaker 1:And you really need to understand, like what you're getting, yeah, and how much risk you're willing to take.
Speaker 2:Yeah, and this is where a conversation with a board certified plastic surgeon like Dr Kim Right Is so important.
Speaker 1:Yeah.
Speaker 2:They can really walk you through all of this.
Speaker 1:Yeah, that's what I was thinking.
Speaker 2:Help you understand the nuances. Yeah, and pick the best option for you.
Speaker 1:Because it seems like there's no one best option.
Speaker 2:Right, it really depends on your individual needs.
Speaker 1:Yeah, yeah.
Speaker 2:And your risk tolerance OK, but going back to Motiva's Femtech promise, yes. I think that's just such a cool feature.
Speaker 1:It sets them apart.
Speaker 2:Yeah.
Speaker 1:Because none of the other brands offer that kind of safety net.
Speaker 2:Yeah, it just seems like they really you know, they trust their product and they want you to feel good about the decision that you're making.
Speaker 1:Exactly. They're putting their money where their mouth is.
Speaker 2:Yeah, by offering that. Okay, so we've talked about the implants a lot. Right but what about the company behind Motiva?
Speaker 1:Okay.
Speaker 2:What can you tell me about Escalishment Labs?
Speaker 1:So one thing that I thought was really interesting is that they're publicly traded.
Speaker 2:Oh, wow On, like the New York Stock Exchange or.
Speaker 1:On NASDAQ.
Speaker 2:Oh, okay, cool. So what does that tell us?
Speaker 1:It means that there's a certain level of transparency and accountability, because all of their financial information is public.
Speaker 2:Gotcha.
Speaker 1:So you can get a good sense of their stability.
Speaker 2:Yeah.
Speaker 1:And they're well capitalized.
Speaker 2:Okay.
Speaker 1:Which means they have a lot of resources to invest in research and development and, of course, to support those warranty programs.
Speaker 2:Yeah, because we were talking about how important those are, they are and how much they differ between companies.
Speaker 1:Right, okay, so they're pretty legit company then yes, very much so okay and I think that's really reassuring for patients yeah because you want a company that's gonna be around for a while yeah, for sure and that's committed to this technology yeah and that prioritizes patient safety and satisfaction.
Speaker 2:Okay, cool. Well, I think this has been like a really good overview.
Speaker 1:Yes.
Speaker 2:Of you know Motive-V breast implants.
Speaker 1:We've covered a lot.
Speaker 2:We've talked about the science. We've talked about, like, the safety features, yes, the different types and how Motive-V stacks up against the competition Right, but I think one thing that we didn't really talk about that Dr Kim mentioned in his videos was the placement of the implant Right, because he was talking about how he prefers to place them under the fascia.
Speaker 1:Yeah.
Speaker 2:Which is different from the traditional under the muscle placement Exactly. So can you explain what that is and why that's important?
Speaker 1:Of course. So traditionally, most breast implants have been placed under the pectoral muscle. Okay, the idea was that the muscle would provide better coverage and reduce the risk of capsular contracture.
Speaker 2:Gotcha.
Speaker 1:But there's been a growing trend towards subfascial placement, which means placing the implant above the muscle but underneath the fascia, which is a layer of connect the muscle, but underneath the fascia, which is a layer of connective tissue.
Speaker 2:So it's still under something.
Speaker 1:It is.
Speaker 2:But it's not under the muscle Exactly.
Speaker 1:And Dr Kim prefers this for Motiva implants.
Speaker 2:Okay, why.
Speaker 1:He believes that Motiva's advanced technology, especially that super smooth silk surface, minimizes the risk of capsular contracture, even with subfascial placement, and there are a few potential benefits to doing it this way.
Speaker 2:Okay, what are they?
Speaker 1:So first off it can mean less pain after surgery oh that's good. And then also it can lead to a more natural look and feel because the implant is closer to the breast tissue itself.
Speaker 2:That makes sense.
Speaker 1:So you kind of get the best of both worlds.
Speaker 2:Okay, so the placement of the implant is just as important as the implant itself.
Speaker 1:Absolutely Cool. It's all about finding that sweet spot, yeah, achieving the look that you want.

Speaker 1:Minimizing the complications.
Speaker 2:Yeah.
Speaker 1:And really customizing the approach based on the patient. Yes and their anatomy and their goals.
Speaker 2:Because everybody's different.

Speaker 2:But what about some of those other things that could happen that are maybe less common?
Speaker 1:Yeah, like what.
Speaker 2:Like late seromas or double capsules.
Speaker 1:Okay, yeah, so those are less common, but they can happen.
Speaker 2:Yeah, how often do those happen?
Speaker 1:It's not super common but, a late seroma is basically a collection of fluid that develops around the implant after surgery. It's usually not serious, but it can sometimes require aspiration or even surgical drainage.
Speaker 2:Okay.
Speaker 1:And then a double capsule is even less frequent.
Speaker 2:Okay.
Speaker 1:And that's when two layers of scar tissue form around the implant instead of just one. Oh, and that can make the implant feel hard and look distorted.
Speaker 2:So kind of like capsular contracture.
Speaker 1:Similar to capsular contracture? Yeah, Gotcha. So how do all these brands handle that in their warranties?
Speaker 2:Yeah, so for late seromas. Motiva's basic warranty covers implant replacement for 10 years.
Speaker 1:OK.
Speaker 2:But it doesn't offer any financial assistance for any additional procedures. Ok, but their enhanced warranty does provide $2,000 for two years.

Speaker 1:Yeah, so if you're worried about that, that might be a good option.

Wow, and they're a little more generous with the financial assistance. Okay, mentor offers $3,500 and Sientra offers $2,000.

Speaker 1:And, interestingly enough, neither of Mentor's warranties specifically covers double capsules.
Speaker 2:Really.
Speaker 1:Yeah, interesting, so you might want to inquire about that if you're considering Mentor.
Speaker 2:Yeah.
Speaker 1:Now Scientra's Platinum 20 provides 20 years of coverage for implant replacement.
Speaker 2:Okay.
Speaker 1:But no financial assistance for this particular complication.
Speaker 2:Okay, so it's all over the place.
Speaker 1:It is, it really is.
Speaker 2:Depending on what company you choose and what warranty.
Speaker 1:Right, and what complication.
Speaker 2:Yeah.
Speaker 1:So it really highlights how important it is. Yeah, how important it is to talk to your surgeon.
Speaker 2:Yeah, for sure.
Speaker 1:To really understand the nuances.
Speaker 2:Yeah, because I would have no idea Right, just going in like blindly.
Speaker 1:Right. So you definitely need to be informed and really think about your risk tolerance.
Speaker 2:Okay. So going back to Dr Kim's videos, he was talking about how motive implants can be inserted through smaller incisions. Is that specific to Motiva or can other implants do?
Speaker 1:that too. So smaller incisions means less scarring, right yeah, and probably a quicker recovery.
Speaker 2:Yeah, for sure. So it's a win-win. Yeah.
Speaker 1:And while it's not exclusive to Motiva, it's something that they emphasize, so their implants are designed to be highly compressible so they can be inserted through those smaller incisions than some other implants.
Speaker 2:Okay, and why is that? Is it because of the material.
Speaker 1:Yeah, it's the combination of the shell design and the gel. So they work together to allow for that compressibility.
Speaker 2:Yeah, that makes sense. So it seems like they really thought about everything.
Speaker 1:Right, it's not just about the implant, it's about the entire surgical experience.
Speaker 2:Yeah, the whole process, the whole process yeah. Well, that's really cool.
Speaker 1:Yeah, and it's definitely something to consider if you're worried about scarring.
Speaker 2:Yeah, for sure.
Speaker 1:You want the fastest recovery possible.
Speaker 2:Exactly Okay, so I think we've talked about pretty much everything.
Speaker 1:Yeah, I think we've covered all the major points.
Speaker 2:Like the technology, the safety features, the warranties.
Speaker 1:Right.
Speaker 2:Different shapes and sizes, even the surgical techniques.
Speaker 1:Right, we went over a lot.
